Use this service to arrange travel vaccinations and get advice for your upcoming trip abroad.
To help the travel nurses to assess your needs, it is important that you fill out this form before your appointment.
You can use this service if you:
- are registered at the surgery
Before you start
We’ll ask you for:
- your first and last name, date of birth, sex, postcode, email and phone number
- if applicable, the details of the person you are completing the form on behalf of
Important: Important Notice
Please note that we require a minimum of 6 weeks’ notice before your date of travel to provide travel vaccinations.
We are unable to offer travel vaccinations if you are travelling within the next 6 weeks.
We cannot accept travel vaccination requests if you do not yet know your confirmed travel date.
Appointments will only be booked, and vaccines ordered, once your confirmed travel date has been provided and it is more than 6 weeks away.
If you are travelling sooner than 6 weeks, or do not yet have a confirmed date, you will need to seek advice from a private travel clinic.
